As a service to our clients, we offer the following assistance:

  • Contact your insurance company directly to verify your benefits, copayment and/or coinsurance.

  • File your insurance claims when provided with the required insurance information.

  • Monitor insurance authorizations, number of authorized visits and prescription expiration date.

  • Fax initial evaluation, progress and discharge summaries to your physician prior to follow up appointments.

  • Assist in obtaining a prescription renewal from your physician.
 






We accept all major medical insurance plans as well as Medicare and are contracted with Blue Shield and Tricare at this time.

  • Payments for charges not covered by health insurance are required at the time services are rendered.
  • Clients are responsible for the payment of deductibles and any coinsurance at the time of service.
  • For your convenience, payments can be made by cash, check or credit card.
  • Please contact Renew Physical Therapy to discuss our fee schedule or any other financial matters.
